Rooted Co-Living pairs safe, stable housing with the services residents need to rebuild their lives. Many services are covered through Medi-Cal's CalAIM initiative.

Sober Living Homes

Structured, substance-free living environments that provide stability and accountability. Residents benefit from peer support, house meetings, and a safe space to maintain their recovery journey.

Recuperative Care / Medical Respite

Short-term residential care for individuals recovering from an acute illness, injury, or surgery who are too ill to recover in a shelter but not sick enough to remain hospitalized.

Housing Transition Navigation

We help individuals identify and secure safe, stable housing. Our navigators assist with applications, housing searches, landlord engagement, and move-in coordination.

Enhanced Care Management

Comprehensive care coordination for residents with complex needs. We connect residents to medical, behavioral health, and social services to ensure whole-person wellness.

Housing Tenancy & Sustaining Services

Once housed, we help residents stay housed. Services include budgeting support, conflict resolution, life skills development, and ongoing case management.

Housing Deposit Assistance

Financial support to help individuals overcome the initial barrier of security deposits and move-in costs when transitioning into permanent housing.

Medically-Tailored Meals

Nutritious meals designed to support recovery and health. Our meal programs address dietary needs for residents with chronic conditions or recovering from hospitalization.

Short-Term Post-Hospitalization Care

A safe environment for individuals discharged from hospitals who need additional recovery time. Includes monitoring, medication management, and care coordination.

Covered Through CalAIM

Many of our services are accessible through California's CalAIM initiative, which expands Medi-Cal benefits to include Community Supports like housing navigation, recuperative care, and enhanced care management. If you or a loved one has Medi-Cal, you may already qualify.
